Vessel approaches exclusion zone too fast and collides with platform

  • What happened?

    A vessel was required to support cargo offloading at a platform.

    The vessel was travelling at 8 knots.

    The handover process from chief officer (C/O) to master was performed 1.1 km from the platform.

    The vessel was traveling too fast to reduce enough speed before it reached the 500 metres (1640 feet) zone.

    The master tried to reduce it to a final rate of 0.6 knots, but it could not be stopped.

    Instead of drifting away from the platform, the master activated reverse steering.

    The vessel collided with a platform.

    It caused platform label damage, a broken support pipe and distorted the boat landing corner.

  • Why did it happen?

    The navigational offshore location procedure was not followed or enforced – the vessel did not follow the 500 m zone entering safety checklist.

    The master was not competent to perform his role.  Technical training was not provided to the crew.

  • What did they learn?

    Confirm the crew are competent – if found not to be so, complete retraining where required.

    Conduct an audit on the 500 m zone re-entry procedure to confirm the safe practice of all vessels approaching a platform.

    All crew should be trained on how to use the controllable pitch propellers (CPP) system in normal and emergency situations.

    Communicate with crew the risk of not following the steering asset/waypoint.
