What happened?
A banksman was caught between two conductor pipes while they were being offloaded from a supply vessel.
He complained about back pain to his room mate, who reported his condition to the medic.
The medic sent him ashore for medical testing. The banksman had suffered serious internal injuries and a fractured pelvis.
The incident was not reported to the rig supervisor, and only discovered 3 days later.

Why did it happen?
The load moved sideways during offloading. The banksman was standing in the path of the moving pipe.
The deck foreman’s position blocked the banksman’s escape path.
No one intervened when they saw the banksman was in an unsafe position.
A lift plan for casing/conductor pipes was available, but a plan for containers was used instead.
The day shift identified the risks in the job safety analysis, but this was not reviewed by the night shift at shift change.
The night shift team underestimated the risk or the potential outcome.

Review job risk analysis:
- when supervisor or any team member changes.
- at shift change.
Report all incidents.
