Unsecured grating falls from tank during lift

What happened?

A starboard crane was being used onboard a vessel.

The crane was repositioning a tank on the aft deck.

A piece of grating on top of the tank fell off whilst it was being moved.

The grating weighed 17kg (37lbs) and it fell 7m (22 ft) to the deck.

There were no injuries – all crew were clear of the area.

Why did it happen?

Only the smaller gratings on the tank container were secured effectively.

The securing clips on top of the tank were not engaged properly.

Only one out of four ‘butterfly’ clips was engaged on the side gratings.

The crew correctly stayed clear of the suspended lift, avoiding the drop zone.

Reiterate the need for a cautious approach when lifting.
