Undiscovered damaged turnback loop causes load to drop to vessel floor

  • What happened?

    Crew were operating a davit crane, with a load consisting of ten 20-litre drums of hydraulic oil (approximately 200-250 litres in total).

    The load was being lifted down to a waiting crew transfer vessel (CTV).

    A failure occurred which resulted in the load dropping out of control down to the CTV.

    The load impacted onto the vessel handrails. No injuries to personnel and no environmental issues occurred.

  • Why did it happen?

    The turnback loop termination on the wire rope failed, causing the load to drop.

    The lifting accessories have not been checked before starting the job, so the damage on the turnback loop termination was not discovered.

  • What did they learn?

    Check all lifting equipment before use. If there is a DOUBT about any of the equipment, DO NOT USE it.

    Ensure the total lifting weight of the load does not exceed the maximum weight allowed.

    Review the lifting procedure and lifting equipment.
