Serious finger injury received while handling heavy shackle pin

  • What happened?

    A team of six workers were re-rigging heavy lifting gear.

    Two riggers were holding the crane hook upright, another three were holding the shackle.

    A sixth rigger was holding the shackle pin ready for insertion.

    As the sixth rigger pushed the shackle pin home, his left index finger was nipped between the head and the body of the shackle.

    This resulted in a serious injury to his finger.

  • Why did it happen?

    Lack of awareness of the hazards and complacency – this was a routine task.

    Rigger was not wearing the correct personal protective equipment (PPE), partly due to poor communication and poor enforcement of the requirements.

    • He should have been wearing the prescribed impact resistant gloves (see image).

    Poor design of the shackle pin and rigging method used – the size and shape of the pin head left little room for the fingers to hold it.

    The work area was restricted and cramped.

  • What did they learn?

    All workers should be wearing the correct PPE for their tasks.

    • Ensure adequate PPE is available at all times.

    Consider introducing easier rigging arrangements involving less manual handling, including the possible use of mechanical aids.

    Investigate ways to discourage complacency with workers when they carry out routine tasks.

    Use a larger area for re-rigging or clear the work area before commencing.
