What happened?
20 x 8 ft concrete (4.8 metric tonnes) mattress was deployed from a barge over an abandoned pipeline.
A diver was away from the dive cage securing tools at the end of a work line.
Diver was instructed to proceed to safe haven.
Dive supervisor instructed crane operator to lower concrete mattress without confirming diver position.
Diver was pinned in soft sea bed by mattress for 10 seconds. Alarm was raised by the diver.
Diver supervisor stopped operations and mattress was retrieved back to deck.
Diver was unhurt but shaken.

Why did it happen?
Diver’s location was not known by dive supervisor before lowering the mattress.
Diver did not confirm he had returned to dive cage as instructed by dive supervisor.
Complacency of diving supervisors:
- Procedures not followed prior to and during dive activities.
- Operation undertaken without functioning scanning/positioning equipment.

What did they learn?
Diver’s positioning must be known at all times.
Crane hook, diver and diver cage to be equipped with location beacons.
Follow the job procedures:
- Overboard the load with the crane slewed away from the divers and diver’s cage in a safe location.
- Stop the load at least 10 metres (33 feet) above the diver’s depth using the crane line out meter for depth.
- Ensure heave compensation is engaged at this point.
- Do not slew the load towards the divers, cage or any mid water tend point without confirming their locations.
