Incorrectly secured cable quadrant falls to deck, injuring crewman

  • What happened?

    On a cable laying vessel, a cable quadrant had been rigged on the bulwarks.

    It was seated in a pedestal mounting and secured via lashings on the outboard side.

    The securing was removed before the deck crane was secured.

    Movement in the cable caused the quadrant to topple and fall to the deck.

    The quadrant struck a crewman in the leg, grazing it.

    Crewman received first aid and returned to work in the next shift.

    Incident was treated as a near miss but had the potential to result in more serious injury.

  • Why did it happen?

    Procedure was not followed:

    • The lashing was removed before securing the deck crane.
    • Normal operations required the deck crane to be secured and taking the quadrant’s weight before removing the lashing.

    Reliance on control measures (the outboard securing) which provided a single point of failure.

    Failure to identify who is in charge for the lifting operation caused confusion and miscommunication.

  • What did they learn?

    Pedestal mounting should be fitted with tipped bolts to secure it directly to the bulwark to reduce the risk of toppling.

    Identify who is charge before starting lifting operations to avoid confusion or miscommunication.

    • A specific person in charge of deck operations should be designated by management.

    Control measures should be reviewed to identify potential improvements.

    • Although certain control measures have worked in the past, they can be improved.

    Ensure all crew are aware of the control measures in place by:

    • Reviewing job safety assessment (JSA) during toolbox talks.
    • Conducting site walk-about before starting work.
