What happened?
An articulated dump truck (ADT) operator fell asleep while travelling to the hard rock dump area to offload.
He drove into the safety berm separating the two haul roads, causing the ADT bucket to overturn on its side.
The operator was not injured. The ADT and berm were damaged.
The site was cleared, and the incident was reported as a ‘minor operational incident’ with a low potential to be investigated by the contractor’s management team.
After a photo emerged, it became clear it was a very serious incident and a full investigation was launched.

Why did it happen?
Operator fell asleep, lost control of the ADT and drove into the berm, damaging it.
Probable cause: Insufficient sleep the night before.

What did they learn?
- Identify the hazard: recognise when you (and others) are too tired to work or drive.
- Do not operate a vehicle when tired.
- Report if you are too tired to drive.
- Seek support if you are having trouble sleeping.
This incident is still under investigation.
