An oil sample from a heat transfer system was taken by opening a difficult to reach a drain valve. The system at 10 bar and 250 °C released suddenly and vigorously hot oil above its flashpoint. Ignition took place after 13 minutes when an operator without antistatic safety shoes & clothing tried to stop the release.
Ask yourself and your crew:
- How can something like this happen here (e.g. on our site)?
- What safety measures (i.e. procedures, controls/barriers) do we have in place to mitigate the risk?
- How do we know the risk controls/barriers are working?
- What improvements or changes should we make to the procedures, controls/barriers or the way we work?
