High waves destabilise crew transfer vessel during transfer
A technician was climbing down a transition piece (TP) ladder into a crew transfer vessel (CTV) when waves reached 1.5 meters (5 feet). The IP was climbing down the ladder onto the CTV when a wave destabilised the CTV, which resulted in the technician hanging from the yoyo. While hanging, the IP hit the part of the structure. The technician did not report pain at the time, but the next day he was in pain and was sent home. It was a lost time injury (LTI) incident. Despite following the right procedures, everything happened quite fast, which did not give the crew time to react. MP pointed out that the design of the TP worked, the IP climbed back up and waited for the right conditions in the safe zone provided by the fenders in the TP.
