Incorrect rigging configuration when using davit crane

  • What happened?

    Technicians were lifting and rigging equipment from a crew transfer vessel (CTV) to a platform.

    They were using an auxiliary hoisting device rigged on the opposite side of the structure, with rope passing through a point on the davit crane (see image).

    The lifting and hook-on point was at height of 2m (6.5ft) over the platform.

    When lifting a 50-100kg (110-220lbs) load, one technician inadvertently touched the crane handle.

    Handle spun at high speed and repeatedly hit a technician’s arm, which became heavily swollen.

    He was able to climb down to CTV and was sailed to shore for hospital checks.

    Fortunately, he did not suffer any fractures and returned to work the second day.

  • Why did it happen?

    The handle spun at high speed due to the forces on the davit crane.

    Incorrect rigging configuration of the auxiliary hoisting device was used in combination with the davit crane (no temporary hoist function when crane is not electrically operated).

    Worm gear cannot withstand horizontal stress during lifting.

  • What did they learn?

    Review all method statements, risk assessments and lifting arrangements to ensure correct rigging configuration when using davit cranes in combination with auxiliary hoisting devices. If necessary, amend and reapprove before starting work.

    Ensure workers are trained and certified in using the auxiliary hoisting devices systems in place.
