Close V-door before unspooling cable drum

  • What happened?

    A crew was preparing for a cementing operation.

    An air hoist cable spool became tangled on the cable drum.

    A rig worker assisted in unspooling and respooling the cable on the rig floor.

    The worker was standing at the V-door, holding tension on the cable while the hoist operator began spooling up the cable.

    The cable got caught at the bottom of the V-door ramp, creating additional tension.

    The cable tightened up and pulled the worker through the V-door, away from the rig floor.

    He slid down 20 feet/6 metres on the pipe slide to the pipe wrangler catwalk.

  • Why did it happen?

    V-door was damaged and remained opened during the task. In normal conditions, it would be closed.

    The rig worker was instructed to hold tension on the cable near the open V-door.

    The thimble eye of the hoist cable got caught at the bottom of the V-door ramp in the pipe wrangler, creating additional unexpected tension.

    The rig worker was not wearing fall protection.

  • What did they learn?

    Re-spooling the hoist line should be performed on the rig floor, and not through the V-door.

    Consider installing bump gate v-doors on rigs.

    Investigate if tensioner devices could potentially eliminate or reduce ‘bird nesting’ (tangling) on the hoist lines.
