Hydrogen vents

Published on 11/03/23 1067 Views

Hydrogen was released by a pressure safety valve (PSV) and ignited, causing damages. The initially installed vent line was dismantled after being folded by high reaction forces during a former release. It was decided to wait until the next turnaround to repair the vent line.

Ask yourself and your crew:

  • How can something like this happen here (e.g. on our site)?
  • What safety measures (i.e. procedures, controls/barriers) do we have in place to mitigate the risk?
  • How do we know the risk controls/barriers are working?
  • What improvements or changes should we make to the procedures, controls/barriers or the way we work?
