While working on a fan outside a shop, an engineer mistakenly switched off the power for both the fan and a freezer inside the shop. In his rush, he did not follow the lockout/tagout (LOTO) procedure. Unaware of the maintenance, a shop assistant turned the switch back on, restoring power to the fan and causing a hand injury to the engineer.
