Release of toxic gas at pesticide unit

Four workers were killed and another three were injured during a large release of highly toxic methyl mercaptan within the plant’s insecticide unit. The sequence of events began with the plant’s shutdown due to a problem with the reactor and within that unit, methyl mercaptan reacts with other chemicals to create insecticide. Operators then tried to restart the unit, however the start-up was unsuccessful because piping within the reaction system was blocked. Hot water was flushed through the piping to remove the blockage. A valve was left open that should have stopped hot water flowing into other piping. This led to the methyl mercaptan release.
