Getting caught in the line of fire means being struck by or caught between two objects or entangled in machinery. This can result in serious injury or even death. Be aware of potential pinch points and don’t stand between moving and fixed objects. This is especially important when you are working alone, the visibility is poor, you are not sufficiently trained or prepared for the task, the equipment doesn’t meet the right safeguards, or if you are feeling tired.

Ask yourself and your crew:

  • How can something like this happen here (e.g. on our site)?
  • What safety measures (i.e. procedures, controls/barriers) do we have in place to mitigate the risk?
  • How do we know the risk controls/barriers are working?
  • What improvements or changes should we make to the procedures, controls/barriers or the way we work?
