Explosion and fire at wastewater treatment plant

An explosion and fire occurred at a waste treatment plant that killed two workers and injured a third. The explosion occurred as the worker used a cutting torch above a storage tank containing highly flammable methanol. Additionally, the incident occurred as the city did not have a programme to control hot work and did not adequately train workers on the flammable and explosive hazards of methanol.
