What happened?
A forklift spotter entered an unsafe zone while a forklift telescoping arm was moving a load to the back of a storage container.
As the load was being moved, the container door unexpectedly swung towards the spotter.
The container door pushed the spotter into the forklift resulting in leg injuries.
-
Why did it happen?
The worker was in the line of fire of the moving container door, which pushed him into the path of the forklift.
The forklift operator couldn't see the spotter, contributing to the incident.
The movement of the container door was not expected upon approach to load the container.
-
What did they learn?
Prior to operating equipment, potential sources of energy near mobile equipment should be secured.
Spotters should maintain a safe distance from mobile equipment, allowing for operator error while remaining in a safe zone.
Having spotters in blind spots significantly increases the risk of being in the line of fire of mobile equipment.
