What happened?
During rigging in activities, the set-up crew installed tarps, around the rig floor, for weather protection using a mechanical lift system. The work was completed, and the mechanical lift was returned.
The next day the rig crew found one tarp had been forgotten. A lifting plan was developed to hang the last tarp with an individual in a harness using the boom hoist.
As the worker was lifted, the line caught on a hook handle or dog ear at the bottom of the doghouse, wedging the worker underneath it. The worker sustained a broken pelvis.
