Worker falls into open deck hatch

What happened?

One of the deck crew fell into an open deck hatch whilst walking backwards on the fore deck.

The hatch's closing lever hit his back on the lower right side.

He hit his back on the hatch lever when falling backwards but did not fall down into the hatch.

This resulted in an injury and the crew member required time off work.

The hatch was open because an engineer had some work to do but it was not guarded or closed when not in use.

A few days later when joining the vessel in the morning, the same hatch was found open again and had been left open and unguarded all night.

Why did it happen?

No protection around to prevent the fall in the hatch as it was not available on board.

Lack of familiarisation and training of a new crew member and lack of instruction about fall risk management on board the vessel.

Lack of awareness from injured person and personnel who left the hatch open.

Provide dedicated barricading onboard each vessel for open hatches.

Clarify the procedure for opening/closing hatches when in use and not in use.

Ensure there is a global action plan for improvement of CTVs health, safety, and environment (HSE) management.

Be aware of the challenges with ensuring familiarisation and training of new crew members during rapid ramp ups of new CTVs company.

Ensure HSE induction training for risks, hazards and working processes.

Care must be paid on HSE awareness and training of new crew members.

A deck crew member was injured after falling into an unguarded open hatch, requiring time off work. Despite the incident, the hatch was left open and unguarded again a few days later.
