What happened?
A team was performing pre-rigging and messenger line installation works on a wind turbine.
A mobile generator was connected to the cable plug leading to the low voltage distribution box, which was linked to the lighting system on the upper platform of the transition piece (TP).
When switching on the generator, the lights on the platform did not turn on.
A worker went to investigate and found an open end cable by the voltage box. When he picked it up, sparks were observed.

Why did it happen?
The cable leading to the distribution box was unterminated (not connected).
Cables were disconnected during commissioning, but it was not documented. This resulted in workers not being made aware of this before starting the job.
