Unsafe practices in waste management lead to fatal dump truck incident

What happened?

A truck driver parked the vehicle in the waste collection area to clean between the tailgate and the inclined bucket of the dump truck.

A temporary support was used to hold the tailgate.

While cleaning, the tailgate released causing a force of impact on the head of the driver.

The impact force resulted in severe head injuries to the driver.

The injured person was evacuated by an ambulance stationed close to the waste collection and segregation area, to the main clinic at the camp.

The driver was pronounced dead by the clinic doctor on arrival.

The driver was cleaning the tailgate of the dump trucks while the jack was in an open position and the engine was running.

The dump truck driver was in the danger zone (in between the tailgate and the inclined bucket of the dump truck).

There was a lack of proper instructions for safe operation of dump trucks prohibiting the use of dump trucks for waste management.
