Oil drum and pallet fall from semi-trailer

What happened?

During transport on a semi-trailer, an oil drum containing 205 litres (45 gallons) of transmission oil and a wooden pallet came free from load restraints and fell onto a public road.

The driver was unaware of the lost load until arriving at the destination.

The oil drum and wooden pallet were pulled to the side of the road by a third party.

The oil drum remained intact. No spill occurred. No-one was injured.

Why did it happen?

Procedures or guidelines for restraining non-standard equipment were not available and there were knowledge gaps for restraining awkward or non-standard loads.

Adjustments were made to load restraint arrangements after pre-move authorisation was signed.

A bunded cage specifically for use in transporting drums was at site but was not used.

Use cages when transporting drums or similar awkward loads to simplify loading and restraining requirements.

Ensure truck to undertake load restraint training in line with the National transport commission load restraint guide, or training in line with local standards/regulations.

Develop field-based load restraint procedures or guidelines for non-standard equipment that is not detailed in rig-specific ‘load lists’ as part of the rig package.

Train personnel using these procedures/guidelines and validate competency.

Provide chain of responsibility training for crew involved in loading/unloading trucks and trailers.
