Near miss due to dropped cable pipe

What happened?

A technician was climbing down from the nacelle.

On the way down he stepped on the cable protection tubes which were stored and secured on the ladder.

The cable protection tubes were mounted with cable ties and two of them fell along the ladder from the mid top tower (cable loop platform) and down to the next level, where one of the installation technicians was working.

No injury was reported, however a nearby technician could have been seriously injured.

Why did it happen?

Cable pipes were not secured as per the procedure.

Work instruction referring to securing the cable protection pipe on the ground was not followed.

Lack of onsite assessment of potential dropped object.

Technicians already climbed up/down with the obstacle on the ladder (cable protection pipe) and they did not immediately fix or remove them.

What did they learn?

Ensure the work team is familiar with the work instruction and performs a last-minute risk assessment before starting a new task.

Perform regular drop surveys on-site focusing on objects/material stored at height with a potential to fall and injure people:

  • If possible, remove objects stored at height; and
  • If not possible, ensure objects are securely attached with two distinct fastening methods.

Raise workers’ awareness on dropped object hazards.

Promote reporting of unsafe situations and conditions.

While climbing down from the nacelle, a technician stepped on cable protection tubes secured on the ladder, causing two tubes to fall near another technician working below. No injuries occurred, but there was a risk of serious injury.
