What happened?

A near miss incident occurred when an adjustable wrench, weighing 0.8 kg (1.8 lbs.), was accidentally dropped from a height of 35 metres (115 ft.) during work on the crown block of a drill rig.

The wrench, which was secured with approved safety wire and a carabiner hook, became detached during the work, falling towards the drill floor.

Why did it happen?

Despite the tools being checked and secured before work commenced, and the presence of a work permit, the wrench detached and dropped.

The investigation highlighted that the carabiner hook's locking nut was not in the proper locking position and tight, leading to the detachment of the wrench.

Always ensure that tools are not only checked and secured before work begins but also throughout the work process to prevent accidents.

When using carabiner hooks, it is critical to check that the locking nut is properly in place and tightened to avoid unintended detachment.

Evaluate alternative securing according to best practice recommendations.

When working at height, always consider the risks and adhere to rules and procedures to minimise risk.
