Worker killed using a mobile elevating work platform

An operator died while using a mobile elevating work platform to cut away roof coverings in a dry dock.

It is believed that the operator died after becoming trapped between a roof beam and the control panel of the platform.

Using elevated work platforms require safe practices.

Before operating:

• be aware of the possibility of entrapment
• conduct equipment pre-check
• survey work zone (is ground stable, level?)

While operating:

• wear adequate fall protection
• avoid overhead hazards
• understand operating procedures and equipment response timing
