What happened?
A heavy equipment operator was walking towards a common meeting area during night time operations.
At the same time a light truck entered the area in the blind spot of the operator.
The operator was run over by the front tire of the light truck and managed to get out of the way of the rear tires.

The hazard associated with the light vehicle - pedestrian interaction was not identified.
There was a lack of vehicle - pedestrian separation in the common meeting area.
Neither the pedestrian, nor the driver saw each other.
