Loose bushings at the blade root end position presents risk

What happened?

Whilst preparing a blade for installation, Technicians identified a loose bushing at the root end position.

There was potential for the bushing to work its way out of its position and fall from the blade during lifting or on the vessel.

This risk is also presented once a blade has been installed to the hub.

Why did it happen?

The sealant on some bushings had been insufficiently applied meaning it could fall from its position following the removal of the blade bolt.

Despite the potential fall distance from this position, risk to people is deemed low as no one is authorised to stand on the transition piece during blade installation.

What did they learn?

Feedback has been provided to the blade factory and design teams, highlighting that adhesive had been insufficiently applied.

A bracket has been created and is being used to prevent bushings from falling outwards during the exchange of the guide pin.
