Two workers were preforming a maintenance task on a lift deck.

They noticed a 230v cable was missing a plug and had electrical tape covering the end.

An electrician was sent to check, they found the cable was live and it was disconnected from its power source.

Electrical nonconformities had not been flagged to the workers before they entered the workspace.

These should have been flagged physically and resolved before the workers were allowed into the workspace.

They should also have been recorded electronically.
