What happened?
On a floating production storage and offloading (FPSO) unit, during the inspection of the elevator in the cabin, the elevator counterweight housing struck the electrician.
After an emergency rescue, the victim died in the hospital onshore.

Why did it happen?
The victim did not take some safety measures, such as switching the elevator to inspection mode, or hitting the emergency stop before entering the elevator shaft.
The victim stood under the counterweight housing while another electrician was operating the elevator.

What did they learn?
All work must follow procedure and permit to work (PTW).
Isolation or safety measures must be taken while entering confined spaces for inspection or maintenance.
Risk assessments and/or hazard identifications must be completed before starting the work.
Ensure personnel understand that they cannot stand in a line of fire.
Effective training must be provided and only qualified personnel can do inspection or maintenance work on special equipment.
