What happened?
During descent from a wind turbine tower, it was discovered by a technician that a ladder connection joint was fitted incorrectly on to the ladder.
This resulted in an excessive gap in the rail connection for the glide rail, creating a potential risk for the glide rail to detach and fall from the ladder system.
There was also a risk of the ladder coming loose, placing extra strain on other connection points.

The ladder connection had not been fitted correctly to the glide rail either during installation or after some maintenance work.
