What happened?

During descent from a wind turbine tower, it was discovered by a technician that a ladder connection joint was fitted incorrectly on to the ladder.

This resulted in an excessive gap in the rail connection for the glide rail, creating a potential risk for the glide rail to detach and fall from the ladder system.

There was also a risk of the ladder coming loose, placing extra strain on other connection points.
