During rigging up on a drilling rig, a worker unintentionally stepped on an unsecured floor plate and tragically fell 7.5 metres (25 ft.) to the ground.

The floor plate, which had not been properly secured, also fell close to the worker.

The worker was equipped with fall protection gear but was not tied off at the time of the incident.
