What happened?
During transfer operations from a Wind turbine generator (WTG) to a Crew transfer vessel (CTV), it was observed by a technician that when a technician transferred from the WTG back to the CTV, the CTV was not pushed on correctly.
The CTV needs to be up against the transition piece (TP) of the WTG to avoid the risk of a crush between the WTG and the CTV.
A crew member needed additional help from a deck hand to keep their balance during the transfer.

Why did it happen?
Boat landing wasn’t in correct position; hence a gap in between transition piece ladder to the bow of CTV.
Deckhand needed to grip the transferee once descended on deck to stop them from falling into the water.
High potential risk of crushing in between TP and CTV if any unexpected movement happened, such as swelling or strong winds.

This incident highlights the failure of client’s and their contractor’s Safe system of work (SSoW).
Ensure sufficient safety awareness to identify potential risks in working environments.
