What happened?
During refuelling of a Crew transfer vessel (CTV), it was observed and raised by the technicians onboard, that the CTV master left the bridge to support refuelling jerrycans while the vessel was still in push-on position, meaning the vessel was not stopped and just up against the quayside.
The technicians stopped the unsafe act and asked the captain to report back to bridge and report his absence.

Why did it happen?
There was a lack of ‘spare’ crew/deckhands which left the vessel short of crew to safely and timely complete the refuelling task.
The vessel master saw an opportunity to help the crew so left his post at a time when leaving the post is highly discouraged.

There is a need for improved communication between teams/crews.
Ensure the CTV captain does not leave the bridge unless there is an emergency on board.
