What happened?

A Crew transfer vessel (CTV) completed passenger transfer with a Service operation vehicle (SOV) on site.

The CTV moved astern and put the pitch to zero on engines.

The CTV Master made radio communication to the SOV and Marine coordination centre (MCC) regarding passenger transfer and carried out some paperwork tasks.

The CTV made contact with a Wind turbine generator (WTG).

The CTV moved ahead and inspected the stern and asset – the damage was minimal.

The Master was distracted by non-essential administrative tasks.

Proper lookout was not being made.

The passenger transfer occurred up wind and current of an asset.

The international regulations for preventing vessel collisions, should be followed at all times, and not paused to carry out non-essential administrative tasks.

Work authorisation did not take into account the position of the passenger transfer (this should not have occurred inside a safety zone of an asset).
