What happened?
A group of contractors was working in the nacelle of an offshore wind turbine installing new cables.
One of the contractors was stripping the outer insulation of a cable using an approved knife.
The technician slipped and sustained a small cut to one of their fingers.
First aid was administered on site and the contractor was instructed to go to a hospital for stitches.

Why did it happen?
The knife being used was not approved for use on the site.
The use of the knife was not mentioned in the Risk assessment and method statement (RAMS).
The contractor was also not wearing the correct Personal protective equipment (PPE) for the task.

What did they learn?
The operation was suspended, and open bladed, fixed position knives were removed from site in accordance with site rules.
The incident was relayed to the wider team during a daily toolbox talk.
New stripper type pliers were selected to replace the fixed blade knives moving forward.
