What happened?
Two Technicians were working at the Cone penetration test (CPT) portal on a connector vessel.
While one technician was using a wrench to recover connector rods from a completed test, there was an unexpected movement in the vessel.
This lead to the wrench and the technicians hand moving downwards on the connector rod and pinching their little finger between the wrench and the portal.
-
Why did it happen?
Unexpected movement caused the wrench held by the technician and the connector rod to slip slightly, leading to the injury.
The technicians hand was in the line of fire, close to the pinch point between the wrench and the connecting rod.
-
What did they learn?
Personnel are to receive more training on correct hand placement for this task.
There was a heave compensated platform on board, whose use would have meant this couldn’t happen, but it was not commissioned for use at the time.
Correct hand protection was being used by the injured person.
-
Ask yourself or your crew
Is the correct hand placement being used for a task, if specified?
Is there a better procedure or other equipment on board that would make completing this task easier or less dangerous? If so, what is it?
Is the technician performing the task aware of any changes in procedure for performing the task?
