Two Technicians were working at the Cone penetration test (CPT) portal on a connector vessel.

While one technician was using a wrench to recover connector rods from a completed test, there was an unexpected movement in the vessel.

This lead to the wrench and the technicians hand moving downwards on the connector rod and pinching their little finger between the wrench and the portal.

Unexpected movement caused the wrench held by the technician and the connector rod to slip slightly, leading to the injury.

The technicians hand was in the line of fire, close to the pinch point between the wrench and the connecting rod.

There was a heave compensated platform on board, whose use would have meant this couldn’t happen, but it was not commissioned for use at the time.
