Cargo handling incident

What happened - icon

What happened?

A cargo handling assistant was manually transporting a piece of equipment.

He moved across a footbridge with a grillage floor.

During the movement, one of the grills detached and he fell from a height of 12 metres (13.12 yards).

The fall resulted in his death.

What happened - icon

Why did it happen?

The grillage floor was fixated poorly using plastic fixings.

The planning/execution of the cargo handling was poor.

There was insufficient information on the composite material of the floor to all involved in its installation process.

The construction and assembly process were poor.

What did they learn?

Establish a plan to substitute all plastic fixing clamps in grillage floors with specific clamps or pins, according to the manufacturer’s recommendation.

Review the cargo handling procedures to promote formal training, especially for supervisors, and improve the program of job on-site training.

Promote a workshop with relevant floor manufacturers and construction/assembly companies to discuss best practices in design, installation, substitution, fixation and maintenance of such structures, and review the procedures with the results of the workshop.

Define the minimum requirements to work on these types of floors, including stages of planning, execution and control/supervision.

Ask yourself or your crew

How can we avoid gaps and oversights during the planning and execution of the cargo handling process?

How can we improve our construction and assembly process to prevent similar incidents occurring at our organisation?

What training programs or certifications should be mandatory for cargo handling personnel, especially supervisors?

What protocols are/should be established for regular inspections and maintenance of grillage floors?

How can we enhance communication and collaboration between floor manufacturers, construction companies, and installation teams?
