Cable support falls off crane to an occupied deck below

  • What happened?

    While performing crane lifts, a cable support (wire rope wear board), measuring 20 inches in length, 4 inches in width, and 2 inches in thickness, and weighing ~8 lbs (3.7kg) fell ~35 feet (11metres) to deck below where workers were present.

  • Why did it happen?

    Carbon steel bolts securing the cable support (wear board) to the crane boom were found corroded and had failed.

    Inspections of the cable support (wear board) securing bolts were not covered in the crane inspection strategy and preventative maintenance task lists.

  • What did they learn?

    Review the bolts securing cable supports (wear boards) and other ancillary equipment to crane booms.

    Uncoated carbon steel bolts should be replaced with bolts of the correct capacity and with corrosion resistance that is suitable for the operating environment.

    Verify that crane inspection strategies and preventative maintenance task lists include a physical and visual inspection of cable supports (wear boards) bolts and bolts supporting other ancillary crane equipment with dropped/fallen object potential.

    Pre-use inspections should be performed by a qualified crane operator or qualified inspector and should include (but should not be limited to) performing a walk-around visual examination of the crane boom and support structure to verify that no visible damage exists.
