Breaker covers found not to be mounted within electrical cabinet

What happened?

After opening an electrical cabinet, a team of technicians realised that breaker covers were missing on 2 x 690v breakers.

They were present in the bottom of cabinet but had not been mounted in the correct location.

The team isolated the cabinet and asked an approved subcontractor to fit the protections back on the 690V breakers.

Why did it happen?

The investigation found that it probably happened during transit to the site.

It is suspected that the covers were not properly fitted to the breakers and so during either transit or initial fitting, they came loose and detached.

It is unclear whether proper checks were performed when the parts were delivered.

The covers needed to be retrofitted by an approved subcontractor.
