What happened?
After opening an electrical cabinet, a team of technicians realised that breaker covers were missing on 2 x 690v breakers.
They were present in the bottom of cabinet but had not been mounted in the correct location.
The team isolated the cabinet and asked an approved subcontractor to fit the protections back on the 690V breakers.
-
Why did it happen?
The investigation found that it probably happened during transit to the site.
It is suspected that the covers were not properly fitted to the breakers and so during either transit or initial fitting, they came loose and detached.
It is unclear whether proper checks were performed when the parts were delivered.
The covers needed to be retrofitted by an approved subcontractor.
-
What did they learn?
Check before installation and before plugging the cable that all covers are in place in this cabinet.
Workers should always follow the electrical safety rules and treat the cabinets as live until zero energy stage is confirmed.
Workers should have access to and wear appropriate Arc Flash Personal Protective Equipment (PPE) according to the task they are performing.
-
Ask yourself or your crew
Do the workers have access and knowledge of the correct Personal Protective Equipment (PPE) used for this task? What is the correct PPE?
Are visual checks performed on the cabinets on delivery of the components?
How do we ensure there is an approved worker on site in case there is a problem?
What would the workers do in this situation?
