Worker falls from height while rigging out

  • What happened?

    Worker fell while rigging out, as they were preparing to move to a new location.

    Lease hand was working on the substructure, folding down wings, when he slipped and fell over the edge approximately 3.5 meters to ground. The worker had crouched down to lift a handrail into place, his hand slipped and he lost his footing.

    Worker was attended by onsite medical, and then sent to nearest hospital. Worker sustained a fractured
    wrist.

    Center grating section of work floor was not  present at time of incident.
  • Why did it happen?

    • When lowering (or raising) wings for rig-out, fall protection was not worn.
    • There was no engineered fall protection system on substructure.

    Contributing factors:

    • JSA and site assessments did not outline all task steps or the need for fall
      protection.
    • Fall protection plan did not identify anchor points, or the requirement for fall
      protection for specific equipment or fall protection training.
    • Pre-job did not specify consistent working at height requirements.

  • What did they learn?

    • Owner will be reviewing all working at height requirements.
    • Review of work plan, JSA for rigging in and out, revising pre-job hazard assessment and communicate to site workers.
    • Company will improve enforcement of Life Saving Rules and fall protection policy.
