A group of visitors attended a meeting about the River Wyre water transfer scheme, held at the Abberystead Outfall Station. The meeting included a demonstration of the station’s operation, with water being pumped over the weir regulating the flow of water into the river. An explosion occurred, causing severe damage and 16 fatalities, roof collapse, and drowning in flooded water chambers. The basic cause was a confined space explosion caused by accidental ignition of methane gas from a coal seam 1200m below.
Ask yourself and your crew:
- How can something like this happen here (e.g. on our site)?
- What safety measures (i.e. procedures, controls/barriers) do we have in place to mitigate the risk?
- How do we know the risk controls/barriers are working?
- What improvements or changes should we make to the procedures, controls/barriers or the way we work?
