When a last line of defence is not working severe incidents happen. A solid last line of defence, like SIL rated interlocks, Pressure relieve devices and Passive barriers, are essential for scenario’s that involve multiple fatalities.
Ask yourself and your crew:

- How do we know the risk controls/barriers are working?
- What improvements or changes should we make to the procedures, controls/barriers or the way we work?
