An uncontrolled release of oil and gas (‘blowout’) occurred in an oil well during a temporary well abandonment procedure. This involved plugging the well with specially formulated cements so it could be left in a safe condition until a production facility arrived at a later date to extract the oil and gas. The escaping hydrocarbons found an ignition source on the drilling rig and caused an explosion. Eleven people died, 17 were injured and 115 people were evacuated. The drilling rig sank within 36 hours of the initial explosion. Nearly 5 million barrels of oil were released, causing massive marine and coastal damage.
Ask yourself and your crew:
- How can something like this happen here (e.g. on our site)?
- What safety measures (i.e. procedures, controls/barriers) do we have in place to mitigate the risk?
- How do we know the risk controls/barriers are working?
- What improvements or changes should we make to the procedures, controls/barriers or the way we work?
