Light gas oil sidedraw line rupture

Published on 08/08/23 742 Views

The light gas oil (LGO) sidedraw from a crude distillation unit (CDU) experienced a catastrophic pipe rupture, releasing a large volume of hot LGO to grade. The hot LGO partially vapourised and formed a large vapour cloud which engulfed 19 company employees. A large plume of vapour, particulates and black smoke travelled across the surrounding area and approximately 15,000 people from neighbouring communities sought medical treatment for a range of ailments such as breathing problems, chest pains, sore throats and headaches.

Ask yourself and your crew:

  • How can something like this happen here (e.g. on our site)?
  • What safety measures (i.e. procedures, controls/barriers) do we have in place to mitigate the risk?
  • How do we know the risk controls/barriers are working?
  • What improvements or changes should we make to the procedures, controls/barriers or the way we work?
