A 30-inch (76cm) expansion joint installed on a recycled gas compressor of a steam-cracking plant failed during start-up. The 36-year-old bellow operated at 3.2 bars (46 PSI) and 32°C (90°F) and showed a 4-inch (10cm) long crack.
Ask yourself and your crew:
- How can something like this happen here (e.g. on our site)?
- What safety measures (i.e. procedures, controls/barriers) do we have in place to mitigate the risk?
- How do we know the risk controls/barriers are working?
- What improvements or changes should we make to the procedures, controls/barriers, or the way we work?
