Disabling safety systems when working with high pressure washers can lead to fatality

Published on 02/09/22 799 Views

During a high-pressure water cleaning task, a worker disabled the dead man switch to be able to reach a difficult area. He lost his balance, the nozzle came out of the bundle, and he was hit by the high-pressure water beam. The worker suffered fatal injuries.

Ask yourself and your crew:

  • How can something like this happen here (e.g. on our site)?
  • What safety measures (i.e. procedures, controls/barriers) do we have in place to mitigate the risk?
  • How do we know the risk controls/barriers are working?
  • What improvements or changes should we make to the procedures, controls/barriers or the way we work?
