The deethaniser overhead line of a Saturated Gas Plant (SGP) suffered a failure at an elbow immediately downstream of a wash water injection point. The release caused a huge vapour cloud, resulting in a massive explosion and fire. The pressure wave from the blast caused widespread damage to houses and businesses with 1km radius of the site. A second release occurred which caused the fire to increase in intensity. The fire was brought under control within 70minutes and was extinguished. The damage to the SGP caused the refinery to be shut down for several weeks, followed by a phased start-up.
Ask yourself and your crew:
- How can something like this happen here (e.g. on our site)?
- What safety measures (i.e. procedures, controls/barriers) do we have in place to mitigate the risk?
- How do we know the risk controls/barriers are working?
- What improvements or changes should we make to the procedures, controls/barriers or the way we work?
