A pipeline carrying pressurised liquified Chlorine began to leak at the juncture where it was connected to the plant's construction. This pipeline was positioned at an elevated location on a pipe-bridge, posing a challenge for regular inspections. Unfortunately, this difficulty in access resulted in the oversight of this critical weak point during the inspection process.
